Jennifer Chisholm
Jennifer Chisholm, LICSW, MLADC, most recently served as a Behavioral Health and Outreach Coordinator at Health Care for the Homeless (HCH) in Manchester, where she oversaw and provided case management, therapeutic and recovery support services; this included both in-clinic care and services provided in encampments as part of the HCH Street Medicine team. Her career has focused on homelessness and substance use, especially among Transitional Aged Youth. She was a Case Manager at a transitional living program for homeless youth, and an outpatient/IOP clinician for adolescents with substance use disorders. Jennifer is an Adjunct Professor of Social Work at UNH, where she teaches Social Work Practice and Field Seminar classes. Jennifer has served on the NH Coalition to End Homelessness Board since 2021.
